Contracted Worker Attestation Process

Purpose:

To meet employment requirement compliance for all contracted workers (HHA, RN) (Recover Care Employees working for Recover Health as contracted employees)

Measurement:

  1. 100% of contracted workers will have a completed Recover Health Contracted Worker Attestation following the process outlined below.

Process:

New Contracted Therapist

  1. Print Contracted Worker Attestation - Recover Care Employees Contracted to work for Recover Health 
  2. Contracted worker or Contractor Representative completes top portion of Recover Health Contracted Worker Attestation
    1. Recover Health Office Location
    2. Date
    3. Worker Name
    4. Discipline
    5. Clinician’s License Number (if applicable)
  3. Contracted worker or Contractor representative sends attestation to Recover Health Branch Leadership, or designee
  4. Recover Health will schedule and complete Contracted HHA/RN Training/Competencies, as applicable
  5. Recover Health Branch Leadership validates clinician license, if applicable
    1. If there are restrictions contact Director of Recover Care
  6. Recover Health Branch Administrator signs acknowledgment agreeing all training was completed and approves contracted worker to be scheduled.
  7. Recover Health Branch Administrator will submit a ticket to "support@recoverhealth.org" to add user in PCC.
  8. Once added, Recover Health TCPA will enter:
    1. License Number
    2. Employee Requirements
      1. Contracted Worker Attestation and date completed
      2. Additional Employee Requirements completed by Recover Health per Attestation
    3. Keep Recover Health Contracted Worker Attestation on file and any office specific trainings completed.

Annual Trainings

Note: Only complete Annual Trainings if specific office/Recover Health trainings are mandatory per contract.

    1. Recover Health Office Location
    2. Date
    3. Worker Name
    4. Discipline
    5. Clinician’s License Number
  3. Recover Health completes employee requirement trainings only when listed as a requirement on the Contract Worker Attestation
    1. Once trainings are completed, enter date the trainings were completed into PCC under requirements
