Starting a Recover Care at Home - Recover Health Partnership

Purpose: 

To live out our mission to create meaningful relationships and expand the service offerings of Recover Health by promoting continuity of care. By combining Recover Health's skilled services and Recover Care at Home's private pay extended hourly services, we can, together, provide a seamless experience and comprehensive approach to serve clients wherever they are in their healthcare journey.

Process:

  1. RCH Account Executive to print off Recover Care at Home - Recover Health New Partnership Checklist to guide process and ensure completion of all pertinent steps to partnership success!
  2. Prior to Kick Off meeting, Account Executive to meet with Account Executive mentor to identify:
    1. Target Market
    2. Key Account Types
    3. Ideal Client
    4. Overall service offerings
    5. Review Collateral
  3. RCH Director to work with Marketing team on 
    1. Updating Recover Care at Home - Recover Health Collateral to reflect specific contact and location information
    2. Updating www.recovercare.org website with new location information
    3. Ensuring marketing team adds new location for monthly newsletter
  4. Recover Care at Home leadership to identify key stakeholders in the associated Recover Health Branch including, but not limited to:
    1. Recover Health Administrator
    2. Recover Health Clinical Manager
    3. Recover Health Account Executive
  5. Recover Care at Home Director to setup 2 hour Kick-Off meeting with the following individuals to follow the Recover Care at Home - Recover Health Partnership Kick-Off Meeting Agenda:
    1. Chief Operating Officer
    2. Recover Health Administrator
    3. Recover Health Clinical Manager
    4. Recover Health Account Executive
    5. Recover Health Regional Director of Operations
    6. Recover Care at Home Administrator
    7. Recover Care at Home Clinical Manager
    8. Recover Care at Home Account Executive
    9. Recover Care at Home Director
  6. RCH AE to setup account review meeting with RCH AE, RCH Administrator, RCH Director, RH AE, RH Administrator, and RH RDO.
  7. Prior to account review meeting, RCH Director to pull accounts data, from Point Click Care and Zoho, currently owned by Recover Health and identify:
    1. All accounts that have had at least 12 Recover Health referrals in the past 12 months - average of 1 referral/month 
    2. All SNF and TCU Accounts 
    3. All Independent Living Accounts
    4. All ILF, ALF, Hospice, or Community Service that has not had any sales activities, ever
    5. All ILF, ALF, Hospice, or Community Service that have not had any sales activity in the past 6 months
    6. All ILF, ALF, Hospice, or Community Service with < 5 SOCs in the past 12 months
  8. Review key accounts with RCH AE, RCH Administrator, RCH Director, RH AE, RH Administrator, and RH RDO and follow Recover Care at Home - Recover Health Account Review Agenda with the goal to walk away with clear account ownership between Account Executives. In an effort to streamline referral source contact with AEs, the primary AE will 'own' the account and the secondary AE will work with the primary AE in any situations where appropriate to contact the referral source directly. 
  9. RCH AE will schedule weekly meeting with RH AE to follow the Recover Care at Home - Recover Health Weekly Account Executive Meeting process and agenda.
  10. Account Executives will update all current accounts in Zoho to identify proper Account Owner, prior to first Weekly AE Meeting
  11. At first weekly AE meeting, AEs will review swapped accounts and identify key points of contact and review key information/history.
  12. RH AEs will invite RCH AE to any pertinent partnership meetings (standing or otherwise) 
  13. RCH AE will review accounts and obtain missing or inaccurate contact information for each assigned account.
  14. RCH AE will update "Recover Care at Home Education" to be applicable to market/area
  15. RCH AE to meet with RH Clinical Manager and RCH Clinical Manager to discuss opportunities for ongoing training and education for RH clinical team as well as next steps for training (identify key stakeholders in each area below); review discharge process;  - ADD CONTENT HERE
  16. Schedule training with the following groups to review the "Recover Care at Home Overview"
    1. RN Case Managers - ADD CONTENT HERE
    2. Internal Therapists - ADD CONTENT HERE
    3. Intake - ADD CONTENT HERE
    4. Team Coordinators - ADD CONTENT HERE
    5. TCPA / Personnel responsible for RN Case Manager orientation - ADD CONTENT HERE
    6. Social Work Team (Internal/Contracted) - ADD CONTENT HERE
